To begin the process, please reach out to our Health Resources Coordinator by phone or email. We will schedule a time to discuss your practice, the communities you serve, and how network membership can benefit both your patients and your organization.

Credentialing & Qualifications

Providers participating in the Northwest Florida Rural Health Network are expected to maintain current licensure and credentialing in accordance with Florida state requirements. This includes active licensure with the Florida Department of Health and compliance with applicable federal and state regulations.

For questions about credentialing requirements or to verify your standing with the network, please contact our office directly. We are happy to assist you through the process.
